Most dental procedures today only require local anesthesia to numb just the affected area. It's a safer approach than general anesthesia: the unconscious state created by putting someone "to sleep" can lead to some unpleasant complications.
But patient comfort involves more than preventing physical pain during a procedure. There's also the emotional factor—many people experience nervousness, anxiety or fear during dental visits. It's especially problematic for an estimated 15% of the population whose dental visit anxiety is so great they often try to avoid dental care altogether.
One option is to use general anesthesia for patients with acute anxiety rather than local anesthesia. This removes them consciously from their anxiety, but they must then be monitored closely for complications.
But there's a safer way to relax patients with high anxiety called intravenous or IV sedation. The method delivers a sedative medication directly into a patient's bloodstream through a small needle or catheter inserted into a vein. The sedative places the patient in a relaxed "semi-awake" state, taking the edge off their anxiety while still enabling them to respond to verbal commands.
Coupled with local anesthesia, they won't experience any pain and very little if any discomfort. And many of the sedatives used also have an amnesiac effect so that the patient won't remember the procedures being performed.
IV sedation does require monitoring of vital signs, but the patient won't need help maintaining their breathing or heart function. And although the medication can be adjusted to reduce any lingering after-effects, a patient will still need someone to accompany them to and from their visit.
For lesser anxiety or nervousness, dentists sometimes prescribe an oral sedative to take just before a visit. This can help take the edge off your nerves and help you relax. With either method, though, sedation can help you overcome fear and anxiety and have a more pleasant treatment experience.

To have a beautiful, healthy smile you’ll need to keep those pearly whites clean and plaque-free. Good dental hygiene, though, isn’t a solo act: It’s a duet best performed by you and your dental health provider. While you’re responsible for brushing and flossing every day, your dental hygienist gives your teeth a thorough cleaning every six months (or more). The American Dental Hygienists Association commemorates every October as National Dental Hygiene Month to recognize both the importance of hygiene and the professionals who assist you in keeping your teeth as clean as possible.
The focus for this emphasis on brushing, flossing and professional cleaning? A slick, slimy substance called dental plaque. This thin film of bacteria and food particles builds up on tooth surfaces after eating and gives rise to infections that cause tooth decay and gum disease. And it doesn’t take long without proper brushing and flossing, for a gum infection called gingivitis to start in only a matter of days. Daily hygiene reduces your risk of that happening: Brushing removes plaque from the broad, biting surfaces of the teeth, while flossing takes care of the areas between teeth that brushing can’t access.
So, if you can remove most of the plaque yourself, why see a dental hygienist? For two reasons: First, while daily hygiene takes care of the lion’s share of plaque, it’s difficult to clear away all of it. Over time, even a small amount of missed plaque can increase your disease risk. However, a professional cleaning that uses special hand tools and ultrasonic equipment can easily clean away this leftover plaque.
Second, some of the soft plaque can interact with saliva to form a hardened, calcified form called calculus or tartar. It can harbor bacteria just like the softer version, and it’s next to impossible to dislodge with brushing and flossing. Again, a trained hygienist with the right tools can effectively break up and remove calculus.
There are also additional benefits that come from regular dental visits to your hygienist. For one, hygienists can provide practical instruction and tips to help you brush and floss more effectively. And, after cleaning your teeth, they can point out areas with heavy plaque and calculus deposits. That can help you focus more of your future brushing and flossing efforts on those areas.
So, a shout-out to all the dental hygienists out there: These dedicated professionals work hard to keep your teeth clean. And a big high-five to you, too: Without your daily commitment to brushing and flossing, your smile wouldn’t be as beautiful—and healthy.

To keep a healthy smile, brushing and flossing your teeth every day should be at the top of your to-do list, along with regular dental visits. Dental visits are usually scheduled every six months when your dental professional will remove any built-up plaque and tartar (hardened plaque deposits) missed during everyday hygiene.
If you've experienced periodontal (gum) disease, however, these dental visits may become even more important toward preventing a re-infection. For one thing, your dentist may want to see you more frequently.
Gum disease is caused by bacteria living in dental plaque, which first infect the superficial layers of gum tissue. Even though the body initiates an inflammatory response to fight it, the infection continues to grow as long as there is plaque present to fuel it. The problem isn't just plaque on the visible tooth surface—hidden plaque beneath the gum line can create deep pockets of infection that can be difficult to treat.
To stop the infection, dentists must manually remove plaque through procedures known as scaling and root planing. Any and all plaque and tartar deposits must be removed, even those deep around the roots, to arrest the infection. This often requires several treatment sessions and sometimes gum surgery to access areas below the gum line.
These types of treatments, especially in the disease's early stages, have a good chance of restoring health to your gums. But because of the high possibility of reinfection, your dentist will need to step up your regular dental maintenance from now on. This could mean visits as frequent as every few weeks, depending on your particular case of gum disease and your dentist's recommendation.
Your dental visits after gum disease may also become more involved than before. Your dentist will now monitor you closely for any signs of reinfection and at the first sign initiate a new round of treatment. You may also need surgical procedures to make some areas around your teeth more accessible for future cleaning and maintenance.
Periodontal maintenance after gum disease helps ensure another infection doesn't rise up to undermine your progress. To paraphrase a well-known quote, eternal vigilance is the price of continuing good dental health.
Your child's permanent teeth come in gradually, starting just as they begin losing their primary ("baby") teeth and not ending until late adolescence or early adulthood. That's when the third molars or "wisdom teeth" close out the process.
Because of their late arrival, wisdom teeth have a high potential for dental problems. With a greater chance of crowding or obstruction by other teeth, wisdom teeth often get stuck fully or partially below the gums and bone (impaction) or erupt out of position. In one study, 7 in 10 people between the ages of 20 and 30 will have at least one impacted wisdom tooth at some time in their lives.
It's not surprising then that wisdom teeth are among the most extracted teeth, to the tune of about 10 million per year. Besides those already diseased or causing bite problems, many are removed preemptively in an attempt to avoid future problems.
But wisdom teeth usually require surgical extraction by an oral surgeon, which is much more involved than a simple extraction by a general dentist. Given the potential consequences of surgical extraction, is it really necessary to remove a wisdom tooth not creating immediate problems?
That's not an easy question to answer because it's often difficult to predict a wisdom tooth's developmental track. Early on it can be disease-free and not causing any problems to other teeth. But as some researchers have found, one in three wisdom teeth at this stage will later develop disease or create other issues.
For many dentists, the best approach is to consider extraction on a case by case basis. Those displaying definite signs of problems are prime for removal. But where there are no signs of disease or other issues, the more prudent action may be to keep a watchful eye on their development and decide on extraction at some later date.
More than likely, your dentist will continue to have an ongoing discussion with you about the state of your child's wisdom teeth. While extraction is always an option, wisdom teeth that aren't yet a problem to dental health may be best left alone.
Although adults are more prone to dental disease, children aren't immune from one particular infection, tooth decay. Some children, in fact, are at higher risk for an aggressive form called early childhood caries (ECC).
There are a number of things you can do to help your child avoid this destructive disease, especially daily brushing and flossing to remove bacterial dental plaque, the underlying cause for tooth decay. It's also important for your child to see a dentist regularly for professional dental cleanings and checkups.
But some of their teeth, particularly the back molars, may need some extra attention to fully protect them against decay. This is because larger teeth like molars have numerous pits and crevices along their biting surfaces that can accumulate dental plaque difficult to remove by brushing alone. The added plaque increases the presence of bacteria around the tooth, which increases the risk of decay.
To minimize this possibility, dentists can apply a dental sealant to "smooth out" those pits and crevices in the molars and make it more difficult for plaque to accumulate. This is a quick and painless procedure in which a dentist brushes a liquid plastic resin or similar material onto the teeth's biting surfaces. They then apply a curing light to harden it into a durable coating.
About one-third of children—mostly those considered at higher risk for tooth decay—have undergone sealant treatment. But the American Dental Association and the American Academy of Pediatric Dentistry recommend this preventive measure for all children between ages 5 and 7, and then later between 11 and 14 when additional molars come in. Although there is a moderate cost per tooth for sealant application, it's much less than the potential expense of treating an infected tooth.
Combined with daily oral hygiene and other preventive measures, sealants can reduce the chances of damaging tooth decay. Keeping your child's teeth healthy is an important part in maintaining their dental health today—and tomorrow.
